About us

smiling girlAt the Multnomah County Community Wellness and Prevention Program, we use population-based approaches to address key factors that contribute to chronic diseases, including poor nutrition, tobacco use, tobacco exposure and physical inactivity.

We work with a wide network of partners and public health allies using evidence- and practice-based interventions to create a healthy, active Multnomah County. 

Our work with the community includes

  • School Initiatives - Working with school districts in Multnomah County to create policies that increase children’s physical activity and increase healthy food choices in schools.

  • Active Living - Working with land use and transportation planners to promote walking and a healthy city infrastructure for everyone in the community.

  • Healthy Eating - Working with retailers and community partners to advance policies that support the availablity and afforability of healthy, local food.

  • Tobacco Prevention – Working across settings to create tobacco-free environments and encourage quitting among adults and youth.

  • Health Inequities – Convening community partners to develop policy solutions to chronic disease inequities experienced by some communities.

We continue to raise the alarm about the impact of obesity and chronic diseases, and sustain the momentum for addressing barriers to healthy living.

History

In 2010, Multnomah County Health Department received a Communities Putting Prevention to Work grant from the Centers for Disease Control and Prevention (CDC). The grant allowed a network of more than 30 community partners to propel existing work forward and to move the Healthy Active Multnomah County Action Plan from vision to reality.

In the last two years, there has been significant progress in making changes to policies, systems and environments throughout the county that will help fight obesity and chronic disease. 

The accomplishments of the last two years build on a decades-long track record of innovation and success. They provide a springboard to continue work on multiple strategies that have long-term promise to put healthy eating, active living and tobacco-free places within reach for all who live in Multnomah County.
